Terminbuchung für Corona-Impfungen:

• für Bestandspatient:innen (wenn Sie noch nie in der Praxis waren, machen Sie bitte zuerst einen allgemeinen Besprechungstermin aus)
• für Patient:innen ab 18 Jahren
• Patient:innen zwischen 12 und 17 mit Einverständnis der Eltern
• schwangere Patientinnen, bitte vorher Abklärung mit Gynäkologen/in ob und wenn ja, wann die Corona-Impfung sinnvoll ist (bitte entsprechendes Attest des/der Gynäkologen/in zum Termin mitbringen)
• bitte ausgefüllten Aufklärungs- und Anamnese-/Einwilligungsbogen (siehe unten, Links in grüner Schrift) zum Termin mitbringen

Auffrischimpfungen mit Biontech führen wir zu folgenden Bedingungen für unsere Bestandspatient:innen durch:

• frühestens 3 Monate nach der letzten Coronaimpfung
• Bestandspatien:innen, die nur einmalig mit Johnson & Johnson geimpft wurden ab 4 Wochen nach der Impfung

appointments for Corona vaccination:

• only for existing patients (if you have never been to the practice, please make a general appointment first)
• for patients, who are at least 18 years old
• patients between 12 and 17 with parental consent
• pregnant patients, please consult beforehand with your gynecologist if and when a Corona vaccination is indicated (please bring a document signed by your gynecologist that he/she supports the vaccination to the appointment)
• please fill out the vaccination information sheet (blue link) and bring it with you for the vaccination appointment
